NHS nurse salaries in England are structured under the Agenda for Change (AfC) pay framework. Understanding the band system, pay points, and additional allowances is essential for Indian nurses planning UK migration. The 2024/25 AfC pay award increased salaries significantly.
The NHS Agenda for Change (AfC) pay framework has bands from 1 to 9. Registered Nurses start at Band 5. Within each band there are multiple pay points, and nurses progress through these points annually. Band 5: Staff Nurse — newly qualified or newly registered from overseas. Band 6: Senior Staff Nurse or Specialist Nurse — typically 2+ years UK experience or specialist skills. Band 7: Ward Manager, Advanced Nurse Practitioner, or Clinical Specialist. Most Indian nurses arrive at Band 5 and can progress to Band 6 within 2–3 years.
Following the 2024 pay award: Band 5: £28,407 (entry) to £34,581 (top). Band 6: £35,392 to £42,618. Band 7: £43,742 to £50,056. Band 8a: £50,952 to £57,349. These are base salaries in England. Wales, Scotland, and Northern Ireland have slightly different pay scales under their own frameworks but broadly similar.
Nurses working in London and surrounding high-cost areas receive an additional High Cost Area Supplement: Inner London: 20% of salary (minimum £5,132, maximum £7,994). Outer London: 15% (minimum £3,699, maximum £5,132). Fringe: 5% (minimum £1,130, maximum £1,763). A Band 5 nurse in Inner London earns approximately £33,539 to £40,575/year — significantly more than outside London.
NHS nurses are paid enhanced rates for unsocial hours: Saturday: time and one third (1.33x). Sunday/Public Holiday: time and two thirds (1.67x). Weeknight (8pm–6am): time and one third (1.33x). Many nurses working full rotational shifts earn significantly more than their base band through unsocial hours payments. A Band 5 nurse working regular weekends and nights can earn £32,000–36,000 annually through base + unsocial hours alone.
A Band 5 nurse earning £28,407: NHS Pension contribution ~7.1% = £2,017. National Insurance ~8% = £2,273. Income Tax (20% above £12,570 allowance) = ~£3,167. Take-home: approximately £21,000–22,000/year, or £1,750–1,850/month. In London with HCAS (earning ~£34,000): take-home approximately £2,200–2,400/month. This is after all deductions including pension, which you receive back upon leaving the NHS or retirement.
